The following is a list of items we suggest that you bring to your medical examination.

• Identification with a photo ID – Passport, Driver’s license, or State issued ID,
• Medical Records (with English translation if applicable)
• Immunization Records (with English translation if applicable)
• Payment for Immigration Physical Exam

The following vaccines are required to complete the USCIS Immigration Medical Examination. The patient must have proof of the vaccination or they are required to receive vaccinations to prevent the following diseases:

• Tetanus and Diphtheria Toxoids
• Pertussis
• MMR (Mumps, Measles, Rubella)
• Hepatitis B
• Varicella, or titers drawn
• Polio
• Haemophilus  Influenza type B
• Any other vaccine-preventable diseases recommended by the Advisory Committee for Immunization Practices
